Professional Care Coordination

 Accompanied™

A New Model for Whole-Person Care Coordination

Coming Soon

Across healthcare, mental health, and ministry settings, individuals often experience fragmented care—especially when the spiritual and moral dimensions of the person are left unaddressed.

Accompanied is a Catholic care coordination model created to bridge this gap.

Coordinated Care. Whole-Person Focus.

Built on a whole-person framework, Accompanied:

  • Integrates clinical, community, and spiritual supports
  • Strengthens continuity of care across systems
  • Supports individuals navigating complex needs
  • Bridges the gap between mental health and faith life

From Fragmentation to Accompaniment

Too often, individuals and families are left to navigate care alone—between therapists, physicians, parish life, and community resources.

Accompanied brings these pieces together, ensuring that care is not isolated, but connected—honoring the full dignity of the person.

Advancing Human Flourishing

This initiative is part of a broader effort to advance:

  • Formation of care providers
  • Coordinated systems of support
  • Faithfully integrated approaches to healing

All rooted in a Catholic understanding of the human person.

👉 Building the infrastructure for whole-person healing.

At this stage, we’re connecting with:

  • Catholic dioceses and parishes
  • Clergy and pastoral leaders
  • Mental health clinicians
  • Catholic ministries and care organizations

If you are part of one of these groups, we’d welcome the opportunity to connect.
